• For eligible Secondary Care Patients.
• For eligible Meds-Only Patients.
• For eligible ER One-Time Only Patients.

Patient Assistance Programs (PAP):

• Utilized for any prescription drug available through a PAP
• Chemotherapy patient assistance programs for oncology patients

Disease State Management Services:

• Coumadin Clinic
• Lipid Clinic

Case Management Services includes:

• Medication Utilization Review (UR)
• Educational Services
• Patient counseling on all new prescriptions
• Drug information resources for patients and clinical staff
• Educational programs for clinical and non-clinical staff
• Preceptor Site for Doctor of Pharmacy students from University of Florida and Nova Southeastern University
