Case Management

Patients referred to Secondary Services will have a needs assessment conducted by case management staff to determine if they will benefit from case management or disease management services. The needs assessment process assists in determining preliminary levels of contact, and the range of services to be provided by case management staff.

Case Management is a collaborative process of assessment, planning, facilitation and advocacy for options and services to meet an individual's health needs through communication and available resources to promote quality cost-effective outcomes (Source: Case Management Society of America). Case Management services provided by Orange County Secondary Services involve a multidisciplinary team approach to meet each patient's needs and program objectives. This team includes:

  • Physicians
  • Nurses
  • Pharmacists
  • Allied Health Disciplines
  • Community Service Agency Administrators
  • Managers and Staff
  • Contracted Services 

Disease State Management includes:

  • An integrated system of interventions
  • Measurements and refinements of healthcare delivery designed to optimize outcomes within a specific population.
  • Aggressive prevention of complications
  • Treatment of chronic conditions. 

The program has a clear understanding of the natural course of a disease a population and the effect of interventions at critical points to delay or prevent morbidity and mortality.
